Making the Call

This call is an opportunity to discuss if we’re a good match. You share what you’re going through and the support you need. After, I’ll explain my approach and address any questions you have. If you decide to proceed, we can schedule your intake appointment right after.

Intake Forms

Before our session, I’ll send you a link to my patient portal for completing intake forms. This helps me prepare thoroughly, ensuring our time together is focused on your story. The portal is also where we can securely communicate, share documents, request refills, schedule appointments, and join virtual sessions.

If you are using insurance, you will also receive a link to Headway.com to create a profile for billing purposes. I am unable to confirm your appointment without their verification.

Initial Consultation

During our session, I’ll deeply listen to your story (having already reviewed your intake forms), ask clarifying questions about your medical history, concerns, and goals. Towards the end, I’ll share my initial clinical impression and potential treatment focus, inviting your feedback. Together, we’ll craft an initial personalized treatment plan, starting with 1-3 changes to monitor your response. I may request labs or screening forms before our follow-up.

I have found that dedicating time to thoroughly understand my patients upfront accelerates our progress. This session is your opportunity to share your story and how symptoms impact your life. I’ll delve into your history, screen for conditions, and outline an action plan that may include tracking, scales, labs, and lifestyle recommendations.

Lab Work & Information Gathering

If needed, I’ll order labs during your consultation to screen for medical conditions and nutritional deficiencies We will review them and any treatment adjustments at our next visit. I may also request records from past providers and connect with them directly. Additionally, I will send symptom screeners before every follow-up visit to track progress.

ADHD Testing & Treatment

Please be aware that a QbTest is required for ADHD diagnosis unless there is documentation of a prior ADHD diagnosis from another provider. I may require a recent electrocardiogram (ECG/EKG) results and blood pressure readings before treatment. Typically, I do not confirm the diagnosis or start stimulant medication during the first consultation. QbTesting is a separate cost from the initial appointment fee.

Follow-up Visits

The initial follow-up, scheduled 2-4 weeks after intake, includes reviewing labs and detailing your treatment plan. Subsequent follow-ups occur every 2-4 weeks for the first 3 months, with long-term patients seen every 1-3 months. These visits involve discussing progress, medication adjustments for effectiveness and tolerability, revisiting psychotherapy insights, addressing concerns, and optimizing your treatment plan. We work collaboratively toward your goals through integrating mind, body, and soul care.

Therapeutic Alliance

At Four Pathways Mental Health, we prioritize building a strong therapeutic alliance to ensure you receive the highest quality of care:
  • Extended Appointment Times: We allocate longer sessions to create a supportive environment where you can feel heard, valued, and comfortable expressing yourself without fear of judgment.
  • Focused Caseloads: By limiting our caseloads, we can concentrate solely on your unique needs. Before each appointment, we meticulously review labs, forms, and assessments to keep our sessions centered on addressing your concerns and achieving your goals.
  • Complimentary 15-Minute Consultation: To foster trust and collaboration, we offer a complimentary 15-minute phone consultation. This initial conversation allows you to connect with us, ask questions, and ensure that our approach aligns with your comfort and expectations before scheduling an appointment.
