This Privacy Policy explains how we collect, use, store, and safeguard information when you visit our website, complete forms, register for programs or webinars, or communicate with us via SMS, email, or phone.
By using our website or submitting a form, you agree to the terms of this Privacy Policy. If you do not agree, please do not use our website or services.
Note About Clinical Records: This Privacy Policy applies to information collected through our website and CRM forms only. If you are a clinical patient, your protected health information (PHI) is governed separately by our HIPAA Notice of Privacy Practices, provided during your clinical intake.
1. Information We Collect
Information You Provide Directly
Identifiers: Name, email address, phone number, and mailing address
Form Submissions: Any information you provide when contacting us, requesting a consultation, registering for a program or webinar, or completing a form
Payment Information: Credit or debit card details submitted when purchasing programs, webinars, or other services. Payment data is processed securely through our payment processor and is not stored on our servers.
Information Collected Automatically
Device and Usage Data: IP address, browser type, operating system, referring URLs, pages viewed, and dates/times of your visits

What We Do Not Collect Through Website Forms
We do not collect Social Security numbers, insurance information, clinical or diagnostic data, or sensitive health information through our website or CRM forms. Clinical information is collected and protected separately under HIPAA.

Four Pathways Mental Health complies with the Telephone Consumer Protection Act (TCPA) and A2P 10DLC (Application-to-Person 10-Digit Long Code) messaging requirements.
Transactional & Services SMS
By checking the transactional consent box and providing your mobile number, you agree to receive SMS messages related to services you have requested, including appointment reminders, scheduling notifications, account updates, service alerts, and other time-sensitive communications.
Marketing & Promotional SMS
By checking the marketing consent box and providing your mobile number, you consent to receive recurring automated marketing text messages, including promotions, special offers, educational content, wellness updates, and information about services or events.

4. No Sharing or Selling of Mobile Data
Four Pathways Mental Health does not sell, rent, lease, or share your mobile phone number or SMS opt-in data with third parties for their own marketing or promotional purposes. Your mobile contact information and consent status are used solely to communicate directly with you.

SMS consent records: Opt-in and opt-out records are maintained for at least four (4) years to demonstrate TCPA and A2P 10DLC compliance
Inactive records: After a reasonable period, inactive data may be securely deleted or anonymized unless retention is required by law

Deletion: You may request that we delete your personal information, subject to any legal retention requirements.

Non-Discrimination: We will not discriminate against you for exercising any of your privacy rights.
9. Children’s Privacy
Our website, educational programs, webinars, and online forms are not directed to children under the age of 13. We do not knowingly collect personal information from children through our website. 10. CCPA – California Privacy Rights
If you are a California resident, the California Consumer Privacy Act (CCPA) and California Privacy Rights Act (CPRA) provide additional rights, including the right to know what personal information we collect, the right to delete, the right to correct, and the right to opt out of the sale of personal information. Four Pathways Mental Health does not sell personal information. To submit a request, contact us using the information in Section 12.
